What to expect at your first visit

Booking your first appointment at Monteluz Dental Specialty Group is the easiest part of starting care. Once you submit the form above, our patient coordinator will call or text you within one business day to confirm a time that fits your schedule. We'll review your insurance benefits, answer any questions about cost or financing, and email you a short new-patient form so check-in on the day of your visit takes just a few minutes.

    Your appointment, step by step

    • A warm welcome. Our front-desk team greets you, verifies your information, and walks you back to a comfortable treatment room.
    • Modern, low-dose imaging. Digital X-rays and an intraoral scan let your specialist see what a visual exam alone cannot, with up to 90% less radiation than traditional film.
    • A thorough exam. Your specialist evaluates your teeth, gums, bite, and jaw joint, and asks about any pain, sensitivity, or cosmetic goals.
    • A clear plan. You'll leave with a written treatment plan, a printed cost estimate, and answers to every question — no pressure, no surprises.

    Insurance, financing, and payment

We accept most major PPO insurance plans and will file claims on your behalf. For treatment beyond what insurance covers, we offer in-house payment plans and partner with CareCredit for low- and no-interest financing. Our team will give you a complete cost estimate before any work begins, so you can make informed decisions for yourself and your family.

    Dental emergencies

If you're in pain right now, don't wait for the form. Call us at 909-567-2024 and we'll do everything we can to see you the same day. Common emergencies include severe toothache, swelling, a knocked-out or fractured tooth, a lost crown, or pain after a recent procedure. The sooner we see you, the more conservative — and affordable — your treatment usually is.
